Do you need to schedule an appointment? You can contact us by texting 970-377-2557 or you can book your appointment online.

Do you need to reschedule an appointment? Feel free to schedule the new date and time online (just put in the “notes” field the date and time of the appointment you want to cancel) or you can contact us by calling or texting 970-377-2557.

Payment Portal

If you’d like to pay a bill, you can do that right here!

Current Patient Updates

Current Patients can complete a health history update to keep us up to date with any recent changes. Please call or text 970-377-2557 so we can send you the forms. If you’ve moved, you can send us a quick note to let us know!

New Patient Packet

Paperwork is now automatically sent to our new patients via text and email after booking an appointment. If you have not received the link to complete paperwork, please call or text 970-377-2557 to request electronic paperwork.

Records release

Have you been to another office and would like to have some records transferred over to us? Please let us know so we can send you a Records Release form to complete. Once the form is complete, we will do the rest! If you have already filled one out with your last office, please have them forward that to us before your appointment.
